Please provide 3 copies of all necessary travel forms, at least 2 weeks prior to the travel examination date; otherwise, we will need to reschedule the appointment.

All vaccines, internal or external parasite treatments, rabies titer tests or any other requirements for the pet, are the responsibility of the person exporting the pet for travel.

All travel documents must be completed correctly for the country of destination, including documents for any connecting or lay over countries (if applicable). I will confirm that all documents completed by Roncesvalles Animal Hospital will be reviewed by me for accuracy.

If travelling by air, I have confirmed with the airline all of their requirements for travel on their airline

If travelling by air or car, I have confirmed with all countries involved of their requirements for travel through their country.

I will not hold Roncesvalles Animal Hospital liable for any issues that may arise before, during or after the travel of this pet.
